Direct Debit is only available for recurring payments. All recurring payments will be debited on the 15^th of the month. I authorise JDRF to arrange for funds to be debited from my/our bank account through the Financial Institution in the amount listed above. I acknowledge this recurring gift will continue on a monthly basis until notice is provided by me to JDRF in writing.
